<p>Eligibility<br />
Candidates can apply and appear for the TISS National Entrance Test for a maximum of three programmes.</p>
<p>(a) A Bachelor’s Degree of a minimum of 3 years duration or its equivalent (under the 10+2+3 or 10+2+4 or 10+2+2+1 year bridge course pattern of study or any other pattern fulfilling the mandatory requirements of 15 years formal education) from a recognised university, in any discipline. OR</p>
<p>(b) Candidates who will complete all the requirements of their final year Bachelor’s Degree examinations by Saturday, June 9, 2012, are also eligible to apply to all the programmes (except the four programmes offered by the School of Health Systems Studies) provided they have successfully completed their first and second years, if the degree course is of 3 years; and the first, second and third years, if the degree course is of 4 years. Candidates applying for the M.A. Education (Elementary) programme should complete all the requirements by May 11, 2012. In such cases, admission to any of the Master’s Degree Programmes of the Institute will be provisional. If a provisionally admitted student fails in the final year examination, the offer of provisional admission automatically stands cancelled.</p>
<p>(c) Candidates applying for the Master of Public Health Policy, Economics and Finance; and Master of Public Health in Social Epidemiology Programmes should have a (i) Master’s degree in any discipline, or (ii) Bachelor’s degree in medicine, paramedical and other four years programme like pharmacy, veterinary sciences, engineering and others.</p>
<p>(d) Candidates applying for the Master of Health Administration and<br />
Hospital Administration should have (i) successfully completed their graduate degree in any discipline, or (ii) Bachelor’s degree in medicine, paramedical and other four years programme like pharmacy, veterinary sciences, engineering and others.</p>
<p>(e) Candidates who are in their final year Bachelor’s degree programme are not eligible to apply for admission to any of the Master’s degree programmes conducted by School of Health Systems Studies, i.e., Health Administration, Hospital Administration; Public Health in Social Epidemiology; and Public Health in Health Policy,<br />
Economics and Finance.</p>
<p>(f) Candidates applying for the Master in Habitat Policy and Practice<br />
should have a Bachelor’s Degree in Engineering, Architecture, Management, Physical Sciences, Geography, Planning, Law, or Social Sciences, with some inclination towards habitat related issues/sectors.</p>
<p>(g) Medical graduates should complete their internship on or before Saturday, June 9, 2012. Otherwise, their candidature/admission stands cancelled.<br />

<p>Candidates from remote places where no Internet and telecommunication facilities are available can get a printed copy of the application form from the Institute.</p>
<p>Unemployed Scheduled Caste and Scheduled Tribe candidates, whose parent’s/guardian’s income is less than Rs. 2 Lakhs and Rs. 1.45 Lakhs, respectively, are exempt from paying the Application Fee. They should apply through the E-application, and if they are from remote places, they can send a request letter without the payment challan and stamped envelope, but with a request letter, a plain self-addressed envelope and copies of Caste and Income Certificates attested by any School headmaster or a Principal of College or a gazetted officer. Requests received without the above Certificates will be rejected and no communication will be sent in this regard.</p>

<p>Mode of Payment</p>
<p>Application Fee (non-refundable) is Rs.950/- (Rs.475/- for SC/ST/PD Applicants)</p>
<p>There are two modes of fee payment. You may choose any of the following options:</p>
<p>SBI Cash Voucher (through any SBI Branch)</p>
<p>In this case, after filling the Application Form a SBI Cash Voucher is generated along with the Application Printout. It contains your name and other details. You have to take this to any SBI Branch within India and make the payment. You have to enclose the IITB copy of the SBI Cash Voucher along with the Application Printout.</p>
<p>Demand Draft</p>
<p>In this case, a Demand Draft of the appropriate amount in favour of &#8216;IIT Bombay&#8217;, payable at Mumbai should be obtained prior to filling of the Application Form. Enter the Demand Draft details while filling the Application Form. The Draft should have the validity of 6 months.</p>
<p>The completed Application Printout, along with IITB Copy of the SBI Cash VoucherOR the Demand Draft (write your name and application number on the back of the DD), and other enclosures (if any), are to be submitted in an envelope to GATE Office IIT Bombay on or before 14 October, 2011</p>
<p>Write &#8220;CEED 2012 Application&#8221; on top of the envelope</p>

<p>Age Limit: Age as on 1st August, 2011 must not be more than 25 years (as per High School Certificate).</p>
<p>For M.P.Ed.:</p>
<p>Candidates who have obtained at least fifty five percent marks in the B.P.Ed degree/ or B.Sc. Physical Education, Health Education and sport/ are eligible for admission.</p>
<p>Age Limit: Age as on 1st August, 2011 must not be more than 35 years (as per High School Certificate).<br />

<p>Candidates have to deposit form fee at State Bank of India.</p>
<p>BPEd. Form fee is Rs. 1500.00 for General and OBC candidates and Rs. 750.00 for SC/ST candidates.</p>
<p>MPEd. Form fee is Rs. 2000.00 for General and OBC candidates and Rs. 1000.00 for SC/ST candidates.</p>

The application form can be obtained from Syndicate Bank. Karnataka University Campus, Dharwad, on payment of Rs. 400/- (Rs. 200/- for SG/ST/Cat-l). (Those who desire to obtain the application form by post are required to send a Demand Draft of Rs. 500/- (Rs. 300/- for SC /ST /Cat-l) (including Rs. 100/- towards postage and service charges) drawn on payable to &#8216;The Finance Officer, Karnatak University, Dharvrad“ along with a sell addressed envelope (A4 size) with affixed.</p>
<p>The words &#8220;Application For Admission to the International Diploma in Reproductive Health Management Course&#8217; should be written on the postal cover addressed to:</p>

<p>The Prescribed fee for Entrance Test is Rs. 500/- for GM and for SC/ST/Cat-l candidates is Rs.250/-. The same should be sent along with the duly filled admission application form &amp; DD/Challan payable in favour of “The Finance Officer Karnatak university, Dharwad” payable at Dharwad”.</p>
